This application form is for BDA Endorsement only. This option is only available in the following circumstances:

• You are submitting a third party publication or resource to request BDA or BDA specialist group endorsement.

If the above does not apply to your resource, you could consider the following options:

• For learning activities please refer to our BDA CPD Approval process.
• For BDA policy statements, position papers or guidance.

Please be mindful that a decision to approve your resource will be based on the information you provide in this application form, so please provide as much information as possible, and complete all fields as indicated. Failure to provide sufficient information may delay or invalidate your application.

Once you have completed the form, please save this and upload below. Please retain a copy of your completed application form and supporting paperwork for your records.

Section 2: Supporting Documents

Please ensure you attach all relevant supporting material below, this should include a copy of the final version of the document or resource but other relevant uploads could include: equality impact assessment, author biographies etc. 

If you have more than 5 files to attach then please forward any additional documents via email to the Approval and Endorsement Team.

By submitting a request for BDA Endorsement, you are confirming that the information you have provided is, to the best of your knowledge, accurate and true at the time of submission and you have read and understood the BDA Endorsement Guidance and the BDA Endorsement Terms and Conditions.
