New NHS mileage reimbursement scheme confirmed following successful consultation

Following a successful consultation, which members contributed to, a new mechanism for NHS mileage reimbursement is to be implemented for members working in England.

The previous mechanism relied on fuel prices rather than the full cost of motoring, which meant many members were left out of pocket as it didn’t reflect the true cost of travelling for work using your own car. As we saw in January 2026, this led to rates decreasing when fuel process fell, despite other motoring costs rising.

We negotiated the new scheme alongside other trade unions with the NHS Staff Council.

While we recognise the new scheme is not perfect, with it not fully meeting the needs of high-mileage drivers or those with older, less fuel-efficient vehicles, it represents a clear and meaningful improvement of the previous system.
